پیوست الف: نتایج پرسش‌نامه

این پرسش‌نامه به منظور بررسی حداقل نیازمندی‌های مربوط به زنجیره تامین شرکت داروسازی ایران داروک که بر اساس روش حداقل استانداردهای زنجیره تامین (CSCMP) استخراج شده، طراحی شده است. در این پرسش‌نامه برای هر نیازمندی ۵ وضعیت اجرایی «کاملا رعایت می‌شود»، «بصورت نامنظم رعایت می‌شود»، «تا حدودی رعایت می‌شود»، «کم و بیش رعایت می‌شود» و «اصلا رعایت نمی‌شود» در نظر گرفته شده است که سطح عارضه بودن نیازمندی به ترتیب وضعیت اجرایی گفته شده افزایش پیدا می‌کند. به عبارتی نیازمندی‌هایی که اصلا رعایت نمی‌شوند به عنوان بدترین نوع عارضه تلقی خواهند شد. جدول زیر، نتایج حاصل از مصاحبه‌های صورت گرفته با مدیران و کارشناسان هر واحد را نشان می‌دهد.
